All appointments and applications must be completed online or by mail for our service areas in Baker, Clay, Duval, Flagler, Nassau, Putnam and St. Johns County.

You are required to make an appointment for ALL services before completing step 2. Applications completed without an appointment identification number will not be accepted for processing. Appointments can only be made for the county where your residence is located. You can make an appointment through the Online Appointment Scheduler or call 904-362-8052. The appointment line is available 24 hours, 7 days a week. Appointments can be scheduled up to 14 calendar days in advance. A live operator will be available for customers who require assistance between the hours of 10 a.m. – 5:00 p.m. Eastern Standard Time. Language accommodations will also be available. A NFCAA representative will contact each individual on the designated appointment day and time to conduct the intake process over the phone. This process addresses the following:
Using the Online FASTRACK System is the quickest way to expedite services (all programs). The customer must upload the required documents into the system by scanning or taking pictures. Applications will not be processed without the following documents. The client must copy the Application ID number that is provided when application is submitted. This will be required. Please do not complete multiple applications. This will delay your processing of services.
After the customer has completed the application and it has been submitted, the Family Support Specialist may request additional information through the Application Tracker in order to process the request for services. The customer will need to provide 1. First and Last Name, 2. Date of Birth and 3. Application Identification Number.
The Family Support Specialist will send an email to the email address provided on the application advising what information needs to be provided. The Application Tracker provides the customer with the ability to log in online and view the status of their application and read any notes or comments made by the caseworker. If needed, the customer can also upload missing or additional documents. Whenever a customer uploads a document to an existing application, an automated email notification is sent to the caseworker responsible for the application.

Please complete the Utility Bill Certification Form if requesting utility services and the bill is not in the customer or household member’s name. The person whose name is on the bill must complete and sign this form. Customers that live in Clay County must complete the City of Green Cove Springs Utility Form.
